Accurate diagnosis matters before treatment begins. Thyroid dysfunction, iron deficiency, and autoimmune conditions produce similar presentations. This overlap is commonly overlooked.
My assessment includes a density evaluation, a hair pull test, and a complete hormonal panel. I also review thyroid function, ferritin, and relevant autoimmune markers. Treating a hormonal problem requires confirming that the problem is in fact hormonal.
How Hormone Replacement Therapy Treats the Follicle
Hormone replacement therapy addresses the imbalance directly rather than managing the symptom at the surface.
Estrogen restoration increases estrogen receptor activity at the follicle and extends the anagen phase. Follicles spend more time growing and produce thicker shafts.
Progesterone restoration reestablishes inhibition of 5-alpha-reductase. Less enzyme activity means less DHT formation at the follicle, which slows and reverses miniaturization in follicles that remain viable.

A perimenopausal patient with fluctuating levels requires a different approach than a patient five years past her final menstrual period. At three months, I assess shedding stabilization and cycle normalization. Reduced daily shedding is the first measurable sign that the hormonal correction is working.
At six to twelve months, I evaluate visible density improvement and new growth against the baseline images. This is where patients see the change they came for.
